Application process

1. The Sponsor and the Apprentice are responsible for submitting their information to the Apprentice/ Sponsor Jot Form.
2. Apprentice must Apply online for a permit to practice. Both the sponsor and the apprentice must participate in an online orientation class administered by the Board. Once steps 1 and 2 are completed and the applications are approved, you will be emailed the Online Apprentice Orientation Documentation.
   All fees are nonrefundable.
3. The Sponsor is responsible for submitting the Apprentice monthly report labeled Sponsor Certification for Apprentice Training. 
4. Note: Sponsors are required to resubmit their information every 5 years.
