• Certificate of Liability Insurance
  Please present the following form to your insurance carrier: Notice to Insurance Carriers Issuing
  Insurance to Persons Seeking Coverage for a Licensed Locksmith Business Certificate Holder (PDF)
• Passport size photo of the applicant
• Report of Employee (PDF)
• Copy of your fingerprinting receipt

Complete the application and send all supporting documentation to:

Additional Requirement for Licensure: Criminal Background Check

The owner or designee is required to undergo a full state and federal criminal history record check through the Criminal Justice Information System Central Repository. There is a separate fee that will be handled by or paid to the fingerprinting location.

As a condition of employment, all employees who provide locksmith services on behalf of a licensed business must undergo the same background check. Employees who do not provide such services are not bound by this requirement.

The Maryland Locksmiths Act empowers the Department to deny a license to an applicant who under the laws of the United States or of any state, is convicted of a:

• Felony; or
  
• Misdemeanor that is directly related to the fitness and qualification of the applicant or licensee to be involved in providing locksmith services;

OR

• Knowingly employs or knowingly continues to employ an individual who, under the laws of the United States or of any state, is convicted of:
1. a felony that is directly related to the fitness and qualification of the employee to be involved in providing locksmith services; or
2. a misdemeanor that is directly related to the fitness and qualification of the employee to be involved in providing locksmith services

License Period: Two-year period from the date of issuance.
